Findings: Surveyor 2 reviewed 10 children's emergency cards and observed the following: * Child 1, Child 2, and Child 3 were missing the name of the first person to be contacted in case of injury or sudden illness of the child. * Child 4 was missing the date of enrollment and the name of the first person to be contacted in case of injury or sudden illness of the child.

Findings: Surveyor 2 reviewed two personnel files and observed the following: * The new staff training form for Staff 1 was not signed or dated by the Director upon completion (start date 07/22/2020).

Findings: Based on direct observation, the Team Leader observed that the driver (Director) of the facility van endangered the health and safety of the children when she did not put the vehicle in park as the kids loaded the vehicle. She started to drive away from the loading area while the kids were still going to their seats. Once the Director recognized the Team Leader on the sidewalk, she stopped the vehicle and put it in park and then got up to check on the kids.

Findings: Per allegation: Based on interviews with children and staff, the Team Leader and Surveyor were told that there was an incident where a child had been told that they would have to drink soap for using swear words. The staff did go as far as having another child make a cup with soap and water in it to show to the misbehaving child. The parent of the child had told the staff that if the child misbehaves that they could use soap and water to wash the child ' s mouth out. The child did not ingest any soapy water.

Findings: During the inspection of the infant room, the Surveyor observed that there is a dry erase board hanging on the wall and within reach of a crib. The dry erase board has a small ledge attached to the bottom of it that also had dry erase markers on it.

Findings: During the inspection of the infant room, the Surveyor observed in front of the exit door that there was a broken tile with missing pieces that showed the exposed cement and another tile that was warped and lifting from what appears to be from some type of water damage.
