Findings: LS conducted a complaint inspection in which it was alleged that children were using a before and after care room, a preschool room and a twos room that is not an approved room on the license. A change of facilities form was submitted in July but the request was denied because the supporting documentation that was required was submitted incorrectly and as of the time of this inspection, correct staffing patterns still had not been submitted to open the room.
