Findings: LS conducted complaint investigation on 06/16/2026. Complaint states that a child fell and bumped his head due to being understaff. LS observed that the center did not notify the OCC within 24hours of the injury of the child that occurred while the child was at the center and resulted in the parents taking the child to the doctor due to a knot on his head. LS inquired how did the child sustain a knot on his head. Teacher informed LS that the child was playing with a ball which rolled near the table. When child stood up he hit his head on the table. A video was not a available because it was more then 24hrs.
