Child #1's health report was 27 days late per 55 PA Code 3290.131. The health form provider by the child's pediatrician was also an outdated version that did not include a statement that "the child is able to participate in child care and appears to be free from congenital or communicable diseases". Child #1 was picked up at 1:30pm on 9/28/23 and taken to the pediatrician for a well visit where the current DHS health form was filled out and the pediatrician was made aware that they had an outdated DHS health form they had been using. Child #1 is no longer a young toddler so her future well visits will now be no more than 1 year apart. |

Provider Response: (Contact the State Licensing Office for more information.) The PA State Police Clearance on file for household member #2 was a volunteer clearance ran on 11/13/2019. I am now aware that household members must have the same clearances as child care staff (not the volunteer one) so that in the future I will not run volunteer clearances on household members but will run regular child care clearances. I ran household member #2's new clearances to comply with CPSL and Chapter 3490 (relating to protective services) on 9/28/2023. They are currently pending review which can take 2-3 weeks as per the email correspondence. I will submit the completed PA State Police Clearance for household member #2 to Certification Representative for review once I receive them. I will be checking the status daily in the meantime so as prompt review of it can take place. I made my "date of correction" 4 weeks in case of any state delay in processing clearances. Staff person #1's FBI was late being completed by 19 days. It was completed and in the staff file before the files were reviewed for the annual inspection.
