Noncompliance Area: In the infant room it was observed that a shelf used in the diapering area for storing supplies was not attached to the shelf brackets allowing the shelf to fall during the inspection.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Noncompliance Area: The file for staff person #1 contains a current health assessment that was completed greater than 24 months following the prior health assessment.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: In the infant room bottles are stored on labeled shelves. However, a filled bottled was observed on the counter and the bottle was not labeled with the child's name.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Noncompliance Area: The file for staff person #2 contains current mandated reporter training completed on 2/20/2020 which is more than 60 months following the prior mandated reporter training completed 2/16/2015.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).

Noncompliance Area: The changing pad on the diaper changing table in the 2 year old room was torn.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: In the 2 year old room there were 3 plastic bags hanging from the cubby hooks and accessible to the older toddlers who frequently still place objects in their mouths.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: In the preschool room an albuterol inhaler was observed in the medication cabinet without the prescription label and without instructions from the individual who prescribed the medication.

Correction Required: A staff person shall administer a prescription medication only if written instructions are provided from the individual who prescribed the medicine. Instructions for administration contained on a prescription label are acceptable.

Noncompliance Area: In the preschool room an albuterol inhaler was observed in the medication cabinet and was not labeled with the name of the child for whom it was intended.

Correction Required: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container.

Noncompliance Area: In the preschool room an albuterol inhaler with an expiration date of September 2018 was observed in the medication cabinet.

Correction Required: Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label.

Noncompliance Area: The file for staff person #1 contained only 5 of the required 6 hours of child care training during the previous 12 month period.

Correction Required: A staff person shall obtain an annual minimum of 6 clock hours of child care training.

Noncompliance Area: The file for staff person #1, provisionally hired on 10/9/18, does not contain a copy of a completed FBI clearance within 90 days of hire as required under the CPSL. Staff person #1, a provisional employee, was observed working alone in the infant room and was not in the vicinity of a permanent employee as required for provisional hire under the CPSL. Facility Person #1 may not work in a child care position at the facility until a completed FBI clearance is obtained.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: The file for child #1 contained a current health report that lacked a statement from the physician regarding communicable disease and the child's suitability to participate in child care.

Correction Required: A health report shall include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease.

Noncompliance Area: In the white medicine cabinet in the preschool room there were 2 packages of acetaminophen gel caps that were not labeled with the name of the person for whom they were intended.

Correction Required: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container.

Noncompliance Area: In the medication cabinet in the preschool room there was a container of Equate pain reliever that expired in 7/2017.

Correction Required: Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label.

Noncompliance Area: The files for staff #1 and #2 contain current health assessments that lack a statement from the physician regarding an exam for communicable disease and suitability to provide child care.

Correction Required: An adult health assessment must include an examination for communicable diseases and the results of that examination. An adult health assessment must include the physician's or CRNP's assessment of the person's suitability to provide child care.
