Please submit the Plan of Corrections via the LMS portal within 10 days. ***The staff was reminded of the license expiration date and to log on to the portal to pay the fees prior to the license expiring. 3 of 3 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was emailed at the time of the inspection. The Emergency Disaster Plan update form was emailed at the time of the inspection. The DES Group Size requirements were observed to be compliant at the time of the inspection. *** Please email the specifications of the Playworld Systems structure on the playground to the Compliance Officer. | |||
