Findings: After a review of 2 staff files, it was determined the following was lacking: Staff 1 (start date 8/19/2019) had a fingerprint clearance card with an issue date of 10/11/2019. After a phone call to DPS, it was revealed Staff 1 fingerprints were returned in August 2019 and were not resubmitted until 10/9/2019; working over one month without fingerprint registration.

Findings: Per allegation and based on documentation, on 11/19/2019, Staff 1 and Staff 2 were supervising children in the 3's room when Child 1 was present. Child 1 received a deep cut on their scalp that required medical attention. Staff 1 and Staff 2 were not aware Child 1 was injured until the parent arrived to pick up the child at 3:25 pm and noticed the child had dried blood in their hair.
