| Findings: Per allegation and based on documentation, on 11/19/2019, Staff 1 and Staff 2 were supervising children in the 3's room when Child 1 was present. Child 1 received a deep cut on their scalp that required medical attention. Staff 1 and Staff 2 were not aware Child 1 was injured until the parent arrived to pick up the child at 3:25 pm and noticed the child had dried blood in their hair. | |||
