Initial Comments: The following deficiencies were observed at the time of Complaint #00159333 investigation conducted on 2/17/26 and are subject to changes pending programmatic review. A telephone call was made to the Complainant on 2/17/26. The Written Document of Corrections is due within 10 days A complete inspection of the facility was not conducted. The following classroom ratios were observed: Infants: 2:9 Ones: 2:9 Twos: 2:14 Twos: 1:8 Threes: 2:17 Threes: 2:15 Fours: 1:12 Fours: 1:13 Fives: 1:8 Three staff members were interviewed during this investigation. The following documentation was reviewed: Classroom rosters. Infant feeding instructions. Emergency Information and Immunization records. Upon completion of the complaint investigation, it was determined from observation, staff interview and documentation that 1 of 2 allegations was substantiated. The other allegation lacked sufficient evidence to be substantiated. The following deficiencies were observed.

Initial Comments: The following deficiencies were found at the time of the Initial Licensing Inspection conducted on 10/8/24, and are subject to changes pending programmatic review. 15 of the 15 fingerprint clearance cards were verified to be valid via the DPS website during the time of the inspection. Please complete the Plan of Corrections via the online Portal within 10 days of receipt of this Statement of Deficiencies. The facility was not licensed at the time of the inspection. Items discussed, but not limited to, were as follows: Diapering areas, staff files, and cleaning and sanitation
