| Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 9/8/25, and are subject to changes pending programmatic review. A full inspection was conducted at this time. 4 of 4 fingerprint clearance cards were verified to be valid through the DPS website. Please complete the Plan of Corrections via the LMS Portal within 10 days of receipt of this Statement of Deficiencies. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The Empower Survey was emailed to the facility. The DES group size was evaluated at the time of the inspection, and observed to not be in compliance. The following was discussed but not limited to: Life saving medication does not need to be locked, only maintained in an inaccessible area to the enrolled children. The metal fence is rusting and the paint is chipping in the outdoor activity area. Installing a 1/2 door on the laundry room door to allow for proper ventilation while maintaining the hazards inaccessible to the enrolled children. | |||
