Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 9/8/25, and are subject to changes pending programmatic review. A full inspection was conducted at this time. 4 of 4 fingerprint clearance cards were verified to be valid through the DPS website. Please complete the Plan of Corrections via the LMS Portal within 10 days of receipt of this Statement of Deficiencies. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The Empower Survey was emailed to the facility. The DES group size was evaluated at the time of the inspection, and observed to not be in compliance. The following was discussed but not limited to: New rule changes and trainings Staff qualifications Choking hazards Director/ designee qualifications DHS website resources

Initial Comments: The following deficiencies were found at the time of the Compliance inspection conducted on 11/5/2024, and are subject to changes pending programmatic review. Senior Compliance Officer- Cara Leyme, MC 2 of 2 fingerprint clearance cards were verified to be valid through the DPS website during the inspection. Please complete the Plan of Corrections via the online Portal within 10 days of receiving this report. Insurance 7/1/2024 FIre-11/8/2024 Sanitation- 8/31/2025 The Empower Survey link was sent to the Director. The DES group size was observed to be compliant.

Initial Comments: A Compliance inspection was conducted on 11/8/2023. The following deficiencies were cited and are subject to changes pending programmatic review. Senior Compliance Officer- Cara Leyme, MC 2 of 2 fingerprint clearance cards were verified to be valid on the DPS website. The Empower survey was requested. Insurance 7/1/2023 Fire 4/5/2023 Sanitation 8/31/2024 Please complete the Plan of Corrections via the online portal within 10 days of receiving this report. The following was discussed but not limited to: Life saving medication does not need to be locked, only maintained in an inaccessible area to the enrolled children. The metal fence is rusting and the paint is chipping in the outdoor activity area. Installing a 1/2 door on the laundry room door to allow for proper ventilation while maintaining the hazards inaccessible to the enrolled children.
