| All equipment and furnishings were not in good repair. Outdoors I observed the large stationary equipment rusting in places, leave holes and sharp edges. One slide was observed with two cracks. One set of steps on the equipment were observed with the padded flooring worn leaving gaps. | Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. Your facility closes on the last Friday of May each year for summer break and reopens Mid-August. You completed a lockdown drill on 3/16/22 and then again on 9/22/22, one should have been conducted in August upon return to maintain the 3 month time period of this requirement. | |||
