Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. During today's visit you provided the current fire inspection which was completed on 2/1/23, it was not submitted to me within 7 days of completion. And it occurred more than a year since the last inspection which was conducted on 1/27/22.

When three year old children and older were in care, the materials and equipment in the activity area was not in sufficient quantity to allow at least three children to use the area regardless of whether the children choose the same or different activities. While in Space 2 I observed the art center with limited supplies, there were markers, two stencils, stamps and a few magazines, no other supplies were accessible to children in care.

All walls and ceilings including doors and windows were not kept clean and in good repair. Throughout the facility in almost every classroom I observed chipped walls, corners of walls were rough where they had been chipped so much it was rough to touch and some metal was showing in one of the classrooms. Paint was observed peeled from the walls, around a soap dispenser and around windows.

All equipment and furnishings were not in good repair. A tricycle on the preschool playground was observed with torn rubber handle covers, a child was observed peeling at during the outdoor observation. Scarves children were using to a music and movement activity in space 1 were observed frayed and torn.
