Findings: In the upper left bedroom on the second floor, there are two 2x2 inch squares cut out of the drywall where a television previously was hooked up. Provider is going to make this room off limits; LS to revise license appropriately. In the bathroom on the main level, there was a second floor leak requiring part of the ceiling and wall drywall to be removed (about 1 square foot). In the dining room, there are two holes in the drywall, one on the upper section of the wall and the other where an outlet still remains. Provider stated all of those drywall issues will be repaired in the next two weeks. LS requested that the lower hole in the dining room where the outlet is be made inaccessible immediately to children in care. This was completed during the time of the inspection. Provider to send a photo of all repaired sections to LS by November 28, 2019.
