Noncompliance Area: In both the School-age and Preschool rooms, in areas accessible to children, observed protruding bolts were observed on easels that could cause laceration or puncture injuries.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: Although the file for Child #1 contained a current health assessment, the previous assessment was completed more than 12 months prior.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.

Noncompliance Area: Although the files for Staff #1 through #5 contained current health assessments, the previous health assessments were completed more than 24 months prior.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: In an area accessible to children in the Slide Room, protruding metal clips were observed along a baseboard heating panel along the base of the rear wall that presents a threat of puncture and/or laceration injuries.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: A ride-on plastic car used by children in the toddler playground was observed to have an exposed circular area where the faux gas cap was located prior to being dislodged. The edging of the circular area was jagged, which could cause laceration injuries.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: In the Pre-school playground, commercial playground equipment required to be embedded lacked sufficient loose-fill at recognizable fall points.

Correction Required: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair.

Noncompliance Area: The file for Staff #1 contained only one of the required two written, non-family references.

Correction Required: A facility person's record shall include two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person

Noncompliance Area: In a hallway, in an area accessible to and traversed by children under the age of six, an unused, unprotected electrical outlet was observed.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: In both the Toddler and Pre-school playgrounds, in areas accessible to children in care, loose aluminum clips and exposed, inward-facing bolts were observed to have exposed, sharp ends that could cause laceration and/or puncture injuries. In the Toddler playground, in an area accessible to children in care, a vinyl trim piece on an out-building was observed to be cracked with jagged, sharp edges, which could cause laceration injuries. In the Pre-school room, a metal baseboard heater in an area accessible to children in care was observed to have sharp edges that could cause laceration injuries.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: In the Pre-K room, in an area accessible to children in care, an approximately 12"x4" plastic bin was observed to be cracked/broken, which could cause a laceration or puncture injury.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: The file for Child #1, who is 2 years and 7 months of age, contained an immunization record that did not include documentation of any of the three doses of PCV 13 vaccine or the initial dose of the MMR vaccine, which, based on ACIP recommendations, should have been completed when child #1 was six months old (PCV 13) and between 12 and 15 months old (MMR) respectively. There was no documentation on file indicating that parent(s) has a strong belief against noted vaccines.

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP.

Noncompliance Area: A chair rail mounted to the wall throughout the Toddler 1B approximately 3' from the floor, in an area accessible to children in care, was observed to have a protruding nail that could cause an abrasion or laceration injury. Also in the Toddler 1B room, an approximately 3.5' x 2' dry erase board mounted to the wall in an area accessible to children, a protruding screw was observed which could cause abrasion or laceration injury. In the Pre-K room, a sharp staple was observed to be protruding from the chair rail in an area accessible to children in care. In the bathroom located in the Toddler 2 room, two rusted, exposed bolts were observed at the base of toilet in an area accessible to children in care, which could cause puncture or laceration injury. One protruding bolt, which is accessible to children in care, was also observed at the base of the toilet in the second bathroom located outside of the gym. In the Preschool room, a round wooden play table was observed to have 4 exposed bolts on the underside of the table in areas accessible to children in care . Also in the Preschool room, two hinged, wooden shelving units used for toy storage were observed to have relatively sharp metal latches on sides of each shelf in areas accessible to children in care. A light switch outside of the gym entrance/exit, in an area accessible to children in care, was observed to not have a plastic protective cover, which caused the live electrical connection to be exposed.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Observed in the infant, toddler 1A, (1-2 year olds) toddler 1B (1-2 year olds), and toddler 2 (2-3 year olds) were receptacles used for the disposal of diapers that were not acceptable hands-free receptacles. Said receptacles were opened by the weight of the diaper being disposed.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Noncompliance Area: The files for child #1 through #4 contained emergency contact and agreement forms that included current parental signatures; however, documents were last acknowledged by a parent's signature more than six months ago.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: A three-tiered plastic storage unit was observed atop a table in the 3-4 year-old room. Said storage unit was accessible to children in care and contained non-child-proof scissors.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.
