In order to correct the violation now, Staff person #2 turned in a updated health assessment form signed on 1/21/2020 and staff person #1 is scheduled their health assessment to be completed by 10/16/20. In the future, the Director will ensure that all staff have health assessments that are not more than one year old and will be renewed every 2 years. The Director will use the tracking spreadsheet to make sure the health assessments are up to date.
