| Initial Comments: The following deficiencies were observed at the time of the 60-Day Monitoring Inspection conducted on November 07, 2025, and are subject to changes pending programmatic review. A full inspection was conducted at this time. One fingerprint clearance card was verified to be valid through the DPS website at the time of the inspection. The Compliance Officers provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The following was discussed but not limited to: *Forms available on website *DCS CBC process for adults/staff *Fire/Emergency drill procedures *Small plastic slide must be placed over resilient surface or removed Please complete the Plan of Correction via the LMS Portal within 10 days of receipt of this Statement of Deficiencies. | |||

| Initial Comments: The following deficiencies were observed during the Initial Licensing Inspection conducted on July 21, 2025, and are subject to changes pending programmatic review. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The fingerprint clearance card for one staff member was verified to be valid through the DPS website at the time of the inspection. Items discussed, but not limited to, were as follows: *Fingerprinting timeframes *Signing in and out (staff and children) *Children’s Emergency, Information, and Immunization Record (EIIR) card procedures *Staff: files, start date, and training (10-Day and Annual) *Second staff member required for 6-10 children *Discipline *Children with Special Needs: Individualized Plan *Mandated Reporting of Suspected Abuse and Neglect *Diapering procedures *Medication (form, storage, labels) *Supervision (nap, outside, and inside) *Menu with specific foods listed *Field trips: anytime leave the licensed area *Fire safety: extinguishers updated yearly and fire drills monthly *No wading pools, burning candles, medications in first aid kit, etc. *Contact the Compliance Officer when any changes are made to the home, or when the hours/days of operation change The Small Group Home was not licensed at the time of the inspection.
