| Initial Comments: The following deficiencies were observed during the Initial Licensing Inspection conducted on August 11, 2025, and are subject to changes pending programmatic review. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The fingerprint clearance card for two staff members and one resident were verified to be valid through the DPS website at the time of the inspection. Items discussed, but not limited to, were as follows: *Touch up paint on metal fence surrounding outdoor activity area *Fingerprinting requirements for 18/older & renewals *Signing in and out (staff and children) *Children’s Emergency, Information, and Immunization Record (EIIR) card procedures *Staff files, start date, and training (10-Day and Annual) *Second staff member required for 6-10 children *Positive discipline & guidance *Children with Special Needs: Individualized Plan *Mandated Reporting of Suspected Abuse and Neglect *Diapering procedures & surface requirements *Medication (form, storage, labels) *Supervision (nap time, outside, and inside) *Menu with specific foods listed *Field trips: anytime you leave the licensed area *Fire safety: extinguishers updated yearly and fire drills monthly *No wading pools, burning candles, medications in first aid kit, etc. *Contact your Compliance Officer when any changes are made to the home, or when the hours/days of operation change The Small Group Home was not licensed at the time of the inspection.
