The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. It was observed that a broken container of large lego blocks was broken and had not been removed and disposed of. Ot was observed that the outdoor sand/water table had a broken lid and it had not been removed or disposed of. The outdoor sand/water lid was removed and disposed of during the visit today.

All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. It was observed that one staff member had expired in their First Aid training on 5/18/23.

It was ovserved that one staff member had expired with their CPR training on 5/18/23.

All staff did not review the center's EPR Plan during orientation and/or on an annual basis with the trained staff. Documentation of the review was not maintained on file. The EPR plan had not been updated since February of 2022. The director updated the EPR plan during the visit today. This violation was corrected during the visit.

Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. In room 2, I observed nine (9) children ages one year though two years being cared for by one staff. Children were napping. An additional staff person was assigned to room 2 during the visit.

Children were not adequately supervised at all times. Per assistant director and staff report: On 2/28/23 a child walked out of the building, unnoticed while in care. Two teachers were assigned to a group of 6 children ages one year to five years. One teacher was engaged with toileting and one teacher was taking a child to the front entrance for pick up. Neither teacher was aware the child had exited the building. It is speculated by staff and the assistant director that the child exited through the side door of the school age classroom, which opens to an unfenced area of the property.
