On multiple occasions, a staff member placed a two-year-old child in corner of a classroom as a form of discipline. The staff member prevented the child from leaving the area by either using a leg to block the child's exit or by placing their body directly in front of the child.

Child was handled roughly. On February 27, 2023, a staff member pulled up a four-year-old child’s arm and jerked the child’s head back with the child’s hair, pulling a portion of the child’s hair from the scalp. The staff member also pushed the child toward the floor.
