Teacher SD completed their medical report 30 days after being employed. Teacher EW's medical statement could not be located during the visit.

This was corrected during the visit by having SD sign the policy.

Sixteen children, ages two and three years of age, were left with one staff person for approximately five minutes while the second staff person assisted a parent in the classroom. The staff did return to the playground.
