|Findings: Reviewed written records on the Magnus electronic system with the registered nurse. The system does not include all items on the Health Inventory form, including lead screening. LS did not review enrollment dates in the system. The nurse made a copy of the Health Inventory form and Emergency form during the inspection. Facility must modify the Magnus system or have the Health Inventory form and Emergency form completed for all children.|
|Findings: Reviewed substitute logs. Reviewed substitute names on the logs that are not the Personnel List. Submit written statement ensuring the OCC will be notified of staff changes within 5 working days and submit all required documentation per regulation.|
|Findings: Facility confirmed that they use the Westview Dining Hall, Clifton Hall auditorium, Yarnall gymnasium, and the Lower School Modular Room 101 and Room 103 for the preschool program. LS observed Walbrooke 007 in use during inspection. Facility no longer uses 2A, 2B, and 4A. Submit Changes in Child Care Facilities form and fire marshal inspection report and/or permit with room approvals. An inspection to measure the new spaces/rooms will be required. Submit written statement ensuring that changes will not be implemented until written OCC approval is received, which results in a revised Letter of Compliance.|
