Noncompliance Area: During renewal inspection on Wednesday, 05/20/26, cert rep observed medication for a child at the facility without a medication log.

Correction Required: An operator is responsible to establish and maintain a medication log if prescription or nonprescription medication is administered.

Description: Pertinent Laws & Regulations/General Health and Safety

Noncompliance Area: In review of health and safety documents, facility's policy addressing shaken baby syndrome, abusive head trauma and child maltreatment, failed to include prevention and identification of child maltreatment.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children.

Regulation: 3270.103/3270.104(a)

Description: Small Toys and Objects/Clean, good repair, proper size

Noncompliance Area: At the time of facility inspection. There was a brown (child's size) sofa in the pre-school classroom. The front area of the sofa was ripped with foam exposed which could pose a chocking hazard to the children in care.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Description: Each staff person meets quals/Aides supervised all times

Noncompliance Area: At the time of inspection, staff #1 was observed working in the infant classroom unsupervised. The staff's file did not contain written proof to support the staffer working in an unsupervised position.

Correction Required: A staff person or a substitute staff person shall meet one of the applicable staff qualifications for the position in which the person is performing. An aide or a combination of aides shall be supervised at all times by a staff person qualified at minimum as an assistant group supervisor.

Regulation: 3270.36(b)(5)/3270.192(2)(iii)

Description: HS/GED + 2 yrs/Exp, educ., training at facility

Noncompliance Area: During the inspection on 8/11/23, the file for staff #1 did not have written proof of 2-yrs work experience. The staffer was observed working alone in the infant classroom.

Correction Required: An assistant group supervisor shall have a high school diploma or a general educational development certificate and 2 years experience with children. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Description: Clean and good repair/Staff assigned to specific children

Noncompliance Area: A child in the toddler classroom at the facility was injured while in care. The child was playing with a toy car that belonged to the child care program. During the investigation, no staff person could identify the child as a member of their primary care group as required by regulation.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Description: Information re: special needs/Insurance coverage information

Noncompliance Area: At time of inspection, child # 3 did not have health insurance policy number and child # 4 did not have the address of release person documented on their emergency contact forms.

Correction Required: Emergency contact information must include information on the child's special needs, as specified by the child's parent, physician, physician's assistant or CRNP, which is needed in an emergency situation. Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Regulation: 3270.131(b)(2)/3270.182(1)

Description: Toddler/preschool: updated health report every 12 months/Initial and subsequent health reports

Noncompliance Area: At time of inspection, child # 1 did not have an updated health assessment documented in their records.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. A child's record shall contain initial and subsequent health reports.

Noncompliance Area: At time of inspection, child # 3 and #4 did not have required influenza immunization documented in their records.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP. Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. The statement shall be kept in the child's record.

Description: Consent for emergency medical care required prior to admission/Consent for administration of minor first-aid required prior to admission

Noncompliance Area: At time of inspection, the parents of children # 1 and #2 did not provide written consent for emergency medical care and administration of minor first aid as required.

Correction Required: A child's record shall contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission. A child's record shall contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission.

Description: Two written references

Noncompliance Area: At time of inspection, staff person #2 &3 did not have the two written non-family references documented in their records.

Correction Required: A facility person's record shall include two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person.

Regulation: 3270.31(e)/3270.192(2)(iii)

Description: Age and Training/Exp, educ., training at facility

Noncompliance Area: At time of inspection, staff person #1 had 7 out of the required 12 hours of annual childcare training documented in their records.

Correction Required: A staff person shall obtain an annual minimum of 12 clock hours of child care training. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Description: Supervised volunteer/Supervision on and facility premises

Noncompliance Area: During time of inspection, the supervising staff person in room #1 left the person identified as a volunteer (#4) unsupervised in room #1 with three young toddlers present for approximately 2 mins.

Correction Required: A volunteer shall be supervised by a staff person who is qualified at minimum as an assistant group supervisor. The requirement for supervision on and off the facility premises includes compliance with the staff:child ratio requirements in §§ 3270.51--3270.55.

Regulation: 3270.78/3270.113(a)(1)

Description: Lighting/Staff assigned to specific children

Noncompliance Area: During time of inspection during naptime period of the day the lighting in rooms # 2 and #3 was extremely dark and not sufficient to be able see and adequately supervised the children in the space while they were napping.

Correction Required: Rooms, hallways, stairways, outside steps, porches and ramps shall be lighted by artificial or natural light. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.
