Noncompliance Area: Child # 1 and child #2 did not have current immunization of influenza documented in record or signed exemption letter from parents documented in children's records.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP. Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. The statement shall be kept in the child's record.

Noncompliance Area: During time of inspection, the facility did not have a current menu posted for the parents review. The menu was for the previous month.

Correction Required: An operator shall conspicuously post the menu at least 1 week in advance or provide a menu to each family.

Noncompliance Area: At time of inspection, child # 4 did not have health insurance policy number documented on there emergency contact form. In addition, child # 1 and #2 did not have address of release person documented on their emergency contact forms.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable. Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: At time of inspection, child # 1 (enrolled 11/20/17) did not have an initial health assessment with immunization status documented in their record.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.A health report shall include a review of the child's immunized status according to recommendations of the ACIP.

Noncompliance Area: Child # 3 (enrolled 12/11/17) did not have a current health assessment documented in their record. ( last health assessment dated 12/6/17)

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.A child's record shall contain initial and subsequent health reports.

Noncompliance Area: During observation of a diaper change in the main childcare space the staff person completing the dipering process did not wash the childs hands after diapering and had to be reminded to clean diapering surface by certification representative.

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered.Diaper changing surfaces shall be cleaned after each use by wiping the surface with a sanitizing solution or by changing a pad or other surface covering.

Noncompliance Area: At time of inspection, staff person # 1 did not have the date that TB screening was read confirming they are free of communicable tuberculosis documented in their record.

Correction Required: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: Staff person # 3 (hired 12/10/18) did not have two none family references documented in their record at time of inspection.

Correction Required: A facility person's record shall include two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person

Noncompliance Area: At time of inspection, the facility did not have an emergency plan that provided for shelter in the event of an emergency posted in the facility.

Correction Required: The facility shall have an emergency planThe operator shall post the emergency plan in the facility at a conspicuous location.

Noncompliance Area: Staff person # 3 (hired 12/31/19) did not have required clearances (state, child abuse and FBI) or reciepts that they are in the process of obtaining clearances documented in record.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 3 may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #3 may not work in a child care position at the facility.

Noncompliance Area: Staff person # 3 (hired 12/10/18) did not have proof of education and/or experience documented in her record to help qualify her for the position of aide at facility.

Correction Required: An aide shall have attained one of the qualification levels specified in 3270.37(b)(1)-(3).A facility person's record shall include verification of child care experience, education and training prior to service at the facility.

Noncompliance Area: At time of inspection, there were two uncovered electrical outlets (extension cords near desk and at cubbie area) in the main childcare space being used by children under 5 years old.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: During time of inspection, it was observed that the facility was not conducting fire drills at least every 60 days are required. ( last three fire drills were dated 9/5/17, 1/5/18 and 1/7/19)

Correction Required: A fire drill shall be held at least every 60 days. Facility persons and children in attendance shall participate in the fire drill. Facility persons and children shall exit the building, weather permitting.

Noncompliance Area: STAFF 1 AND STAFF 3 DID NOT HAVE A CURRENT HEALTH ASSESSMENT AT THE TIME OF THE RENEWAL INSPECTION.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.
