| Initial Comments: The following deficiencies were found at the time of the the Compliance inspection conducted on February 4, 2026, and are subject to changes pending programmatic review. Two of Two fingerprint clearance cards were verified to be valid via the DPS website during the time of the inspection. The DES group size was determined to be compliant at the time of the inspection. The Emergency Disaster Contact form was completed during the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. Please complete the the Plan of Corrections via the online Portal within ten days of receipt of this Statement of Deficiencies. Items discussed, but not limited to, were: -Renewing license via the Portal - Anniversary Application in “Applications” or “Application History”, -Renew license before 7/31/2026, -Review all staff files for expiring fingerprint cards. | |||
