| Initial Comments: The following deficiencies were found at the time of the Compliance inspection conducted on January 15, 2026, and are subject to changes pending programmatic review. A full inspection was conducted at this time. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. Two of two fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Please complete the Plan of Corrections via the online Portal within 10 days of receipt of this Statement of Deficiencies. The DES group size was observed to be compliant at the time of the inspection. The Emergency Disaster Contact form was completed during the inspection. The link for the Empower survey was emailed to the facility following the inspection. Items discussed, but not limited to, were: -Storing items in the bathroom, -Renewing license via the portal - Anniversary Application in "Applications" or "Application History", -Renew license before 2/28/2025, -Review all staff files for expiring fingerprint cards. | |||

Compliance Officer is Bianka Hains Compliance Officer Supervisor is Peggy Kraus Two fingerprint clearance cards were verified as valid through the DPS website. The Plan of Corrections is due within 10 days of receipt of this Statement of Deficiencies. The following was discussed, but not limited to: *Diaper changing sinks must be next to the diapering surface, *Lesson plans and schedules must be posted in each activity area, *Children's allergy/modified diet postings must not be accessible to the general public, *Emergency plans must be dated for the current 12 months, *Refrigerators must be maintained at 41°F or below, *The Infant Playground must be maintained in a clean condition and free from hazards, *Staff medications must be locked and stored separately from children's medications. | |||
