Findings: Supervision Concerns. This provider self-reported that a child was left inside a classroom for approximately 10-15 minutes while the rest of the children went outside to recess. During the investigation, video footage was reviewed and it confirmed a child was left in the classroom alone. Resolution: The two teachers involved were terminated and the provider implemented a 2-hour Active Supervision technical assistance for all staff at the facility. All children were properly supervised at the time of investigation.
