The staff members stated they do not wash the children's hands at the diaper changing handwashing sink. *The Surveyor's discussed the citation with the Program Lead.

Findings: On the playground, there was one tricycle without pedals and two tricycles that each lacked a pedal.

Findings: Based on Surveyor #2's observation, the bathrooms located in the 2's room, the exhaust fans were not working after the switches were turned on.

Findings: Based on surveyor's review and an interview with the director, it was found that child 1 was not signed out on 1/6 with only a time of departure stated. Child 2 was lacking a time of departure on 1/6. Child 3, child 4 and child 5 were not signed out on 1/6.

Findings: Based on surveyor's review and an interview with the director, it was found that in the 3/4's room there were 20 children present but only 19 on the roster. Staff stated that child 6 just arrived at 10:00 and the time was 10:36. Child 6 had not been recorded in.

Findings: Based on surveyor's observation, it was found that the toddlers were in a buggy to go outside for a walk and then were observed playing on the outdoor play area. This occurred over the course of an hour and a half. No drinking water was taken out by staff when leaving the room. *Surveyor spoke to the director who brought the children's water cups to the staff.

Findings: Based on surveyor's observation, it was found that in the infant room the room arrangement did not allow for staff to see the children who were sleeping in their cribs. There were cabinets and cribs placed to block children off from going in the crib area. There were mattresses standing up at the backside of the cabinets which made the barrier even higher. Staff playing with children on the floor area would not be able to see the children in the cribs.
