Plan: POC established 8/26/25: Owner Crystal will post a posting that is a reminder for safe sleep practices. Program will create a document having staff sign it daily when they enter the infant room of dos and don'ts with safe sleep - a checklist. Provider will have a meeting with Ms. Anna and give written warning. Moving forward Crystal will implement a system that leads to termination of staff if requirements cannot be met. POC established 9/23/25: Crystal will have a discussion her mother, Ms. Janet who was with the infants. Crystal will move Janet to the older children and have her take Safe Sleep again. |
