Noncompliance Area: There was a large plastic bag hanging from the diaper changing table in the infant room that was accessible to the children.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: Infant room staff stated they have been warming bottles directly in the microwave because their bottle warmer was broken.

Correction Required: Bottled formula may not be heated in a microwave oven.

Noncompliance Area: Staff person #1, date of hire 2/4/2020, Staff person #2, date of hire 1/28/2020, staff person #3, date of hire 11/12/2019 and staff person #4, date of hire 12/23/2019, are all working in child care positions at the facility without their National Sex Offender Registry verification. The legal entity does not have an approved waiver to hire provisionally.

Noncompliance Area: There was a can of Febreeze in an unlocked cabinet under the sink which was accessible to the children, in one of the preschool room bathrooms.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: All of the staff have completed American Life and Health Foundation first-aid. This is an online first-aid and not approved by OCDEL. None of the staff have current approved first-aid certification.

Correction Required: Competence in first-aid is demonstrated by the completion of training by a professional in the field of first-aid. First-aid training shall be renewed on or before expiration of certification or every 3 years, as applicable. One or more facility persons competent in first-aid techniques shall be at the facility when one or more children are in care.

Noncompliance Area: There was a tube of diaper ointment in the young toddler room that was in an unlocked cabinet within the reach of children.
