2016-09-13 16N - Records Pending After multiple requests, the facility did not produce tracking sheets that would show how many children were present at exact times in each classroom with specific staff members. The records produced, which include CACFP meal count sheets and electronic time in/time out logs for children, indicate different attendance numbers on the same day. Where the electronic logs showed 12 infants in one classroom, the CACFP meal records show 15 infants in the classroom on the same day. A consistent, accessible attendance system needs to be in place and kept on record, so as to know where children and staff are located in the center throughout each day. These records need to be able to be produced upon request by either the administrator or a designated staff member in charge when the administrator is not available.

Infants must be placed into a crib on their backs upon falling asleep unless otherwise requested with medical documentation. During a follow-up visit, two infants were found sleeping in bouncy seats. Both incidents were corrected at the time of the visit.
