On 4/29/19 at 8:16 am there were ten children signed into the one-year-old room with one teacher and at 4:12 pm there were nine children with one teacher. On 4/29/19, there were 11 children signed into the three-year-old room with one teacher from 9:33 to 9:37 am. On 4/30/19 there were 11 children signed into the three-year-old room from 8:25 to 9:06 am. On 5/1/19 in the one-year-old room, there were seven children signed in with one teacher at 7:53 am and nine children signed in with one teacher at 8:05 am. From 8:28 to 8:34 am there were up to 14 children signed in with two teachers. On 5/1/19 in the three-year-old room there were 11 children signed in with one teacher from 9:05 to 9:35 am. On 5/1/19 in the two-year-old room, a staff with no qualifications was alone from 12:32 to 12:56 pm and there were no staff signed in from 12:56 to 1:33 pm. On 5/2/19, in the one-year-old room, there were seven children and one teacher at 7:52 am and eight children with one teacher from 7:54 am to 8:00 am. On 5/2/19, from 1:54 to 2:26 pm a staff with no qualifications was alone in the four-year-old room.

Corrective Action: Ensure proper staff-to-child ratios and group size requirements are followed, and direct supervision of children is provided at all times through the assignment of qualified staff. Ensure toddlers are cared for in an area physically separate from old children except during the first and last 90 minutes of the day. On April 25, 2019, a staff member who is qualified as an intern and not allowed to be alone with a group of children, was alone in the one-year-old room with seven one-year-olds during naptime. Another staff member who is qualified as an intern and not allowed to be alone with a group of children, was alone in the three-year-old classroom with five three-year-olds. This staff member also did not have a background check for the center due to being under the age of 18. According to attendance records, on April 24, 2019, 18 children, the youngest being one-year-old, were in a classroom with three staff from 8:31 am to 9:06 am. Submit one week of attendance records for all classrooms to OCCL by May 7, 2019.
