A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. One of seventeen children's records reviewed did not have documentation of parent receipt of the summary of NC Child Care Law.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Room 303/Technical closet was unlocked. Stored in the room was lavender floor cleaner, a drill, round up, comet and ant killer. In the children's bathroom in space #3 there was lavender floor cleaner and comet with bleach stored in the counter drawer.

A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In space #10 authorization for a ProAir asthma inhaler was expired.

The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. There were pieces of paper and plastic debris in outdoor space #3. There were three plastic bags along the fence line in outdoor space #1.

Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The most recent documented playground inspection for the outdoor spaces 1, 2, and 3 was 1/6/2023.

Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. Staff hired on 4/25/22 had a medical report on file dated for 4/26/22.
