Koinonia Christian Center Church Ministries is a God-ordained, spirit-filled place of worship located in Greenville, North Carolina intent upon making an impact on the world. We aim to inspire worship and praise of our Lord and Savior, Jesus Christ, and involvement in the world through ministry and outreach. The fabric of our church is the Word of God, and our members span numerous ethnicities, socio-economic backgrounds, livelihoods and ages. As our name suggests, we believe in koinonia or as it is commonly known, fellowship. Through the fellowship of believers, anointed leadership, and declaration of the truth of God’s word, KCC is indeed a church on the move by, for, and through the power of God.

A safe indoor and outdoor environment was not provided for the children. In the outdoor space #2 the lining of grass was exposed, the sand next to the fence was receding creating a space for entrapment under the fence, and in outdoor space number three plastic stepping logs were cracked and broken. In outdoor space #1 there was a large active ant hill near the fence line.

Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Two of three intern's emergency information forms did not include a hospital preference, and one of three intern's emergency information form did not include a date of completion.

The depth of the loose surfacing was not based on critical height of the equipment. On the large preschool playground mulch surfacing under the swings and fall zones of the stationary climber measured less than one inch.

The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. A signed acknowledgement was not on file for staff M. Barton hired on 1/18/24.

A violation was found for which there is no item number. In classroom space #2 a staff member was observed during diapering routines. The staff did not remove both gloves and wipe their hands and the child's hands after removing the child's soiled diaper. This is a violation of sanitation rule .2819 (c).

Food substitution was not of comparable food value or recorded on the menu prior to the meal or snack being served. Children were served blueberry muffin, mixed fruit, and milk for breakfast. Today's menu reflected french toast sticks, mixed fruit and milk for breakfast.
