Description: Phillips, Jacqueline is a Group Family Day Care in Staten Island NY, with a maximum capacity of 16 children. |Brief Description: Applicants for renewal of a license must submit to the Office at least 60 days in advance of the expiration date of the license the requirements specified in 416.2(d).|

|Brief Description: Each program must hold two shelter-in-place drills annually during which procedures and supplies are reviewed. Parents must be made aware of this drill in advance|
|Brief Description: The program must retain on file in the program a medical statement, on forms furnished by the Office or approved equivalents, from a health care provider for each person residing in the home.|
|Brief Description: The group family day care home must report to the Office: any change affecting, or which reasonably might be expected to affect, those portions of the building and property in which the program is operating or which are used for the children's egress in the case of emergency; any change in household members; and any other change that would place the home out of compliance with applicable regulations.|
