I observed a bag of fertilizer/soil in the outdoor learning environment.

Before staff members walked children off premises for play or outings, the center did not obtain written permission from the parent of each child. Seven (7) out of the nine (9) children enrolled did not have a current off premise permission form.

Over-the-counter medicines were not in their original containers or administered as authorized in writing by parent, physician or authorized health professional. I observed a topical ointment for one (1) child with an expired permission form.

The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. I observed trash around a table in the outdoor learning environment.

All staff records, except financial records, were not made available for review. The staff file for A.J was not on site and available for review.

Staff required to receive on-going training had not completed the required number of hours according to their education and experience. P.P did not complete the required eight (8) hours of on-going training, today I observed only two (2) hours on file.

Each child was not immunized as per Article 6 of Chapter 130A and an immunization record was not on file before or within 30 days after admission. One (1) child did not have their immunization records within the required timeframe.
