| Findings: During a review of emergency forms, it was found that three emergency forms do not include the address of the child's physician. Two of these children are siblings. Please have the respective parents add the missing information on their child's emergency form, initial and date it for the addition of the information. It was also discovered that the emergency form for a one year old child was missing. Please send a copy of the emergency cards with the added information as well as a copy of the missing emergency card to OCC for the correction of this non-compliance. | |||
