About the Provider

Description: Outer Limits is located in Central Tucson on Fort Lowell between Country Club and Alvernon. We draw families from the local neighborhood and, due to Ft. Lowell’s status as a convenient East-West thoroughfare, families from as far away as Vail, Sahuarita and Oracle. We currently enroll children from two years through fifth grade. Outer Limits offers full day toddler, preschool and kindergarten programs as well as before and after school programs for elementary school children. School age children are safety transported to and from the following schools: Davidson, Cragin, Presidio and Academy Adventures Midtown.

Findings: -Per the allegation in AZ00175009, AZ0017527 and AZ0017528, and based on staff interviews, facility documentation, medical documentation and photos, the following was substantiated; it was determined that the health and safety of Child 1 was placed at risk of harm on 7/20/2021 when Child 2 bit Child 1. The bite broke the skin on Child 1's back. Staff did not witness the bite as indicated in a report written by Staff 2. The report stated that the bite had to have happened earlier that day because there was no other time that Child 1 was around children that were hurting them. -Surveyor 1 observed two children in the Four Year Old Activity Area with their heads completely covered with their blankets while they were napping. Staff 1 uncovered the children's heads during the investigation.

Findings: Based on staff interviews and facility documentation, Child 1 was bitten on their back by Child 2 on 7/20/2021. The bite broke the skin. Staff documentation of the incident stated that the bite was not noticed until Staff 1 and Staff 2 checked Child 1 for a possible reaction to a snack that Child 1 ate that day. The written report stated that when Staff 2 was checking for a reaction, they noticed a bite mark on Child 1's back and that it had to have happened earlier that day because there was no other time that Child 1 was around children that were hurting them. Classroom staff did witness two other bites which Child 1 sustained that day. The documentation indicates that Staff were not aware of a third bite until they checked Child 1 for a possible reaction to something that they ate, and could not say for certain how it occurred.

Findings: Surveyor 1 observed a teacher in the two year old room with 7 children present. The teacher reported not having a roster with them because it had been left outside with a second two year old teacher who was on the toddler playground with 7 children. The roster listed 14 children present.

Findings: Two staff files were reviewed. The documentation of the training required within 10 days of hire in Staff 1's file did not have the facility director's signature.

Findings: Surveyor 1 observed two teachers with twenty-three 4 and 5 year old children on the playground. One teacher opened the classroom door to allow a child to enter the classroom to use the bathroom while the teacher watched from the doorway. As a result, the child was in a 0:1 ratio when they went into the classroom.

Findings: Surveyor 1 observed 2 teachers with 14 two year old children on the toddler playground with no drinking water accessible to children. There were 13 three and four year old children with one teacher on the scooter yard with no drinking water accessible to children.

I have used this provider for less than 6 months

My child came to Miss Helen’s 3 year old class in the beginning of 2023. In the 90 plus days that she has been there she has been nothing but happy every day about school. She is so proud to show me things in the classroom that she has created and tell me what they have been learning about ( dinosaurs, clouds, etc.).

At home, she actually teaches me things about manners and how to behave (“Miss Helen says cover your mouth to cough”, “Miss Helen says use a tissue”, “Miss Helen says say please”. )

Helen has been great to update me on my daughter’s day to day behavior and accomplishments. This is not something I got at the last facility, but something that is very appreciated. I also love that there are cameras in the classroom so I can observe first hand how things are going. I see Helen and how she interacts with my daughter and her classmates.

I could not be happier with my decision to enroll with Outer Limits.
