Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 5/12/26 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. Items discussed, but not limited to: playground safety, bathroom supervision, lesson plans, staff training, and rule changes.

Initial Comments: The following deficiencies were found at the time of the compliance inspection conducted on 5/12/25, and are subject to changes pending programmatic review. 2 of 2 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The Empower Survey link was emailed to the facility. The DES group size was observed at the time of the inspection. Please complete the Plan of Corrections via the online portal within 10 days of receipt of the Statement of Deficiencies.

Initial Comments: The following deficiencies were found at the time of the annual Compliance Inspection conducted on May 13, 2024, subject to changes pending programmatic review. A full inspection was conducted at this time. Three of three fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Inspection Item Expiration Dates: Insurance: 07/01/24 Fire: 08/29/25 (State) Items discussed, but not limited to: -Put N/A on child emergency card if second parent/guardian is not listed -Pay License renewal fee prior to end date -Resubmit DCS Background Checks if no results received -Refrigerator must maintain temperature of 41 degrees or colder -Medicated items not allowed in First Aid Kit utilized for children -Updated Entrance Letter and DCS Direct Service form -Forms and rule set available on AZDHS website -Licenses are now valid for one year The Plan of Correction was not accepted at the time of the inspection.
