Noncompliance Area: The young toddler child care space lacked a written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency posted in the child care space.

Noncompliance Area: Staff #5 lacked a tuberculosis screening by the Mantoux method at initial employment. Employment date for staff #5 was documented as being 5/30/20 and tuberculosis screening accepted at employment was dated 4/20/18.

Correction Required: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Noncompliance Area: Staff #1 lacked mandated reporter training completed every 60 months. Training dates observed were 2/18/15 and 6/4/20. Staff #2 lacked mandated reporter training completed every 60 months. Training dates observed were 1/22/15 and 3/3/20. Staff #3 was observed having a only receipt of their current Pennsylvania State Police clearance on file and lacked a current DHS FBI clearance. Staff #4 lacked an National Sex Offender Registry (NSOR) certificate.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff #3 and #4 may not work in a child care position at the facility until all required clearances are obtained.

Noncompliance Area: The outdoor play space lacked 9 inches of loose-fill under the embedded equipment in compliance with the recommendations of the United States Consumer Product Safety Commission.

Correction Required: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair.

Noncompliance Area: The infant room was observed having a boppy under a crib mattress to elevate the mattress during sleep times and lacked a doctors note stating this is an appropriate infant sleeping position.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Noncompliance Area: A toxic plant identified as a Peace Lilly was observed in the young toddler child care space and accessible to children.

Correction Required: Toxic plants are not permitted in a child care space.

Noncompliance Area: The children's sink located in the young toddler child care space was observed having a hot water temperature of 128 F.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110 F.

Noncompliance Area: The older toddler and young toddler child care spaces lacked a written plan of daily activities posted in the group space.

Correction Required: The written plan of daily activities shall be posted in the group space.

Staff #5 was observed having a hire date of 1/28/19 and emergency training completed on 3/17/19. Staff #6 was observed having a hire date of 8/6/18 and emergency training completed on 9/19/18.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: Staff #2 signed a disclosure statement dated 1/15/19 verifying a Pennsylvania Child Abuse clearance was applied for but was observed having a Pennsylvania Child Abuse clearance dated 5/3/19. Staff #3 was documented as having a hire date of 5/11/19 but observed having a disclosure statement from previous employment dated 4/19/15. Staff #4 lacked a date on the signed disclosure form. Staff #6 documentation was observed having a hire date of 8/6/18 and a volunteer Pennsylvania State Police clearance dated 4/11/18.
