A copy of the Notice of Inspection Rights was provided at the time of the inspection. 4 of 4 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Initial Comments: The following deficiencies were observed at the time of the compliance inspection conducted on 1/3/2025 and are subject to change pending programmatic review. Compliance Officer: W. Manzelmann The Compliance Officer reviewed 4 staff files. The fingerprint clearance cards for 3 of 4 staff ( 1 staff was under 18) were verified to be valid through the DPS website at the time of the inspection. The following assessments were completed: The Empower Program Assessment was emailed to the director, DES Group Size Evaluation. The following was discussed: Any chairs manufactured with slots for safety straps should have safety straps attached.

Initial Comments: The Team Leader observed that a blue rag was placed on the diaper changing area in the 1's and 2's room. The Team Leader also observed that the wood diaper changing surface in the 2's and 3's room was rough to the touch

Initial Comments: The Team Leader observed that the plumbing connections behind 2 toilets in the west toilet room had build up of discolored material and the tile underneath the sinks had dirt build up. The Team Leader also observed that a toilet bowl in the east toilet room had dirt build up.
