| Findings: Supervision Concerns and Health and Safety Concerns. There were concerns regarding supervision, smoking, and unknow individuals being on-site. During investigation, a child was unattended in a separate room, cigarette buts were littered throughout the front yard and the provider acknowledged vaping occurs on the premise. Additionally, there are no background clearances for all residents over the age of 13. Resolution: Provider obtained background checks for individuals over 13 who will be onsite. Additionally, all evidence of smoking or tobacco use was removed from the premises, and the provider was informed that children must be kept in one main room to ensure supervision is not compromised. | |||
