Initial Comments: The purpose of the inspection was to conduct a Complaint Investigation for Case #00063060 on September 18, 2023. An Annual Compliance Inspection was also conducted. Ratios observed were: *2:9 Ones *2:11 Three/Fours *1:10 Four/Fives There were seven staff members interviewed onsite during the compliance inspection and complaint investigation. Documentation observed included the following: Child and staff sign in/out records, Emergency, Information and Immunization Record (EIIR) cards, and two staff files. Upon completion of the complaint investigation it was determined from documentation and interview that the allegation lacked sufficient evidence to be substantiated. Initial Comments: The purpose of the inspection was to conduct a compliant investigation for case # 63060 on August 29, 2023. A full inspection was not conducted. Senior Compliance Officer Brenda Alubowicz Ratios observed: Pre-K 2:10 Pre-K 2:10 1/2 year old's 2:9 There was one staff member interviewed during this investigation. Upon completion of the complaint investigation, it was determined from observation and interview that 2 of 2 allegations were substantiated. Initial Comments: No deficiencies were observed during the Change of Service inspection conducted on August 04, 2023, adding Infant children and an Infant Classroom to the license. Items discussed, but not limited to: -Label bottles with identifiers -Post license and main postings in an area where Parents will be able to view -Diapering sink water must measure between 86 and 110 degrees for hand washing -Do not allow any items to block the classroom exit -Tummy time shall be documented as one of an infant’s daily activities Compliance Officer: Ryan Mapes Classroom 118 is licensed for 5 Infants based on the classroom only having one exit (R9-5-601.1).
