Findings: Based on the Surveyors' observations, the following hazards were accessible to enrolled children in the outdoor activity area: * The rubber molding was cracked and peeling with exposed rusty metal on 2 of 2 square, blue picnic tables and on the blue picnic bench facing the play area. * There was rusty, corroded metal at the base of the metal grating on the front part of the drinking fountain. * The rubber molding was missing with exposed rusty metal on the second, third, and fourth steps of the large climbing structure, as well as the top platform near the entrance to the curved slide. * There was chipped paint with exposed, untreated wood at the base of the wooden storage shed. * Repeat deficiency from 12/01/17 and 12/6/18.
