Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Eight (8) outlets in the cafeteria did not have covers on them. This program includes a Pre-k component so all outlets in licensed spaces must be covered.

The depth of the loose surfacing was not based on critical height of the equipment. The surfacing on the large second playground used by the older children had spaces where just a tarp was present including under slides and climbing structures. New rubber mulch is needed for children to continue play on this structure.
