Noncompliance Area: The fire drill log observed on file at the child care facility was observed not to have documented a fire drill being completed within the last 60 days. This is evidenced by the most recent dated fire drill documented being dated 9/18/17.

Correction Required: A fire drill shall be held at least every 60 days. Staff persons, volunteers and children in attendance shall participate in the fire drill and shall exit the facility, weather permitting.

Noncompliance Area: The diaper changing mat and multiple rest mats were observed to be torn on the edges exposing the children in care to the foam material contained on the inside of the mats. The foam material can be torn by the children in care to have a diameter of less than 1 inch in diameter creating a choking hazard to the children in care.

Correction Required: Pea gravel and other materials with a diameter of less than 1 inch may not be used in spaces where infants or toddlers receive care.

Noncompliance Area: The file for staff person #1 was observed not to contain documentation of staff person #1 being trained in the facility's emergency plan at time of hire.
