Noncompliance Area: In Room 8 (play space) three marbles and a plastic Ziploc bag were observed. In the kitchen area, empty plastic grocery bags were observed in the middle cabinet below the microwave which is accessible to children.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: The larger refrigerator in the kitchen area did not include a thermometer to ensure proper temperature (45 degrees or lower) is maintained.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: A cloth hand towel, not labeled with a child's name, was hanging on a hook by the sink in the bathroom.

Correction Required: Cloth towels and washcloths shall be labeled with the child's name, used by only the named child and laundered weekly. The operator shall arrange a laundry schedule with the parent.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: In the bathroom, toxics that were accessible to children were observed on the sink/counter (hand sanitizer and toothpaste), a shelf mounted to the wall (shampoo, conditioner, body lotion, deodorant, and toothpaste), a small shelf/table next to the shower (shampoo), in the shower (body wash) and the back of the toilet (spray cleaner with bleach, OdoBan disinfectant spray, Lysol wipes, Purell sanitizing wipes. In the playspace area, baby wipes were observed in the closets. In the changing room, Lysol wipes on top of the dresser were accessible to children. Baby wipes were in the drawer of changing table and in the closet were observed to be accessible to children. In the kitchen, a Swiffer mop containing Swiffer cleaning solution was observed to be accessible to children.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Description: Contaminated trash in closed plastic-lined receptacle

Noncompliance Area: An open garbage receptacle in the kitchen was observed to contain partially eaten food, paper plates, napkins, and drink containers.

Correction Required: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles.

Description: Above ground window opening restricted to 6"

Noncompliance Area: Windows above ground floor in the following spaces were observed to open more than 6 inches: Windows in the designated play space (Room 8), changing room, main room and kitchen area.

Correction Required: Windows or doors above the ground floor that open directly to the outdoors and are accessible to children shall be constructed, modified or adapted to limit the opening to 6 or fewer inches.

Description: Building Surfaces

Noncompliance Area: In the Room 8 (play space), hooks, screws were observed to be protruding from the walls and the trim around the door was observed to have several nails protruding. The trim around the door was not flush to the wall, creating a possible pinch-point. A light-switch plate on the left wall was observed to be cracked with sharp edges. A television was placed on the floor, presenting a tipping risk. In the changing room, the frame around the entry door was cracked, exposing splintered wood. A dresser in the room was not anchored to the wall, presenting a tipping hazard. A crib/changing table unit was not assembled properly (missing several screws), rendering it unstable. In both Room 8 (play space) and the main room and kitchen area, cords on the window blinds were accessible to children, presenting a strangulation risk. Strings loosely affixed to the wall, and a cord dangling from a camera were also observed presenting a strangulation risk. In the main room, the cord to the television mounted on the wall was loosely affixed, presenting a strangulation risk. In the kitchen area, the top drawer on the right side of the sink/countertop area a sharp knife accessible to children was observed. The right drawer below the microwave contained a sharp corkscrew accessible to children, and in the middle drawer below the microwave several sharp knives accessible to children were observed.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
