Noncompliance Area: In the preschool room, observed a three tier book case which was unsteady and wobbled when touched. In the outside play space, observed a red and blue tricycle which had a crack on the plastic seat causing a pinch point.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: The fee agreement on file for child #4 lacked the date on which the fee is to be paid.

Correction Required: An agreement shall specify the date on which the fee is to be paid.

Noncompliance Area: The most recent child service report on file for child #2 was not completed within the past six months and was due in the file prior to regulatory suspension date of 3/6/20. Most recent child service report on file is dated 5/14/19.

Correction Required: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in 3270.124(f) (relating to emergency contact information).

Noncompliance Area: The emergency contact information on file for child #1 lacks a health insurance policy number. The emergency contact information on file for children #3 and 4 lacks a health insurance policy number and the address of individuals designated by the parent to whom the child may be released.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable. Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: The older toddler room lacked a conspicuously displayed medical emergency transportation plan.

Correction Required: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.

Noncompliance Area: The file for child #4, lacked an initial health assessment within 60 days of enrollment and was due in the file prior to regulatory suspension date of 3/6/20. There are no health assessments on file.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.A child's record shall contain initial and subsequent health reports.

Noncompliance Area: The files for child #1 and #2, both who are older toddlers, lack an updated health assessment within the past 12 months and health assessments were due in the file prior to regulatory suspension date of 3/6/2020.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.A child's record shall contain initial and subsequent health reports.

Noncompliance Area: The files for children #1, 2, 3, and 4 lack verification that immunizations are in accordance with the schedule recommended by the ACIP.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: There was more than 24 months between the previous and most current and updated health assessment on file for staff #1.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: The files for staff #3 and 4 lack a TB screening within 12 months prior to working in a child care position.

Correction Required: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: The files for staff #5 and 6 lack one written, nonfamily reference from individuals attesting to the person's suitability to serve as a facility person.

Noncompliance Area: There was more than 60 months between the previous and most current mandated reporter trainings on file for staff #2. Dates are as follows: 6/29/15 and 8/30/20.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Noncompliance Area: There was more than 60 months between the previous and most current State Police and Child Abuse clearances on file for staff #2. Dates are as follows; State Police - 3/30/15 and 8/27/20, Child Abuse- 3/30/15 and 9/1/20. The file for staff #3 lacked a National Sex Offender Registry certificate by 7/1/20 and the current State Police clearance has record for control but rap sheet is not attached. There is no National Sex Offender Registry certificate on file. The file for staff #4, with start date of 8/17/20, lacked a National Sex Offender Registry certificate on or before initial start in a child care position. There is now an NSOR on file and dated 8/24/20. The file for staff #6, who has lived outside of PA within the past 5 years, lacks an Ohio State Police, Ohio Child Abuse, and Ohio Sex Offender Registry check on or before initial date in a child care position. There are no out of state clearances on file.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Facility Person # 3 and #6-- may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #--- may not work in a child care position at the facility.

Noncompliance Area: Observed sanitizer spray in the infant room and older toddler room which lacked a label that specifies the contents.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: On the back wall of the preschool room, observed a two inch hole. In the preschool room, observed a 12 x 8 ft. alphabet carpet. Most of the carpet was dirty, all 4 corners were frayed and ripping, and the middle gathered causing a tripping hazard to the children in care.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
