Children were not adequately supervised at all times. On March 10, 2022, two staff members were responsible for a group of children on the playground. One staff member was seated and was standing while the staff members were talking to each other. The staff members failed to move around the playground to maximize their ability to supervise the children.

Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. On March 9, 2022, a staff member pulled a five-year-old child to them and held the child's upper arms while speaking loudly and in a harsh tone to the child.
