Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 5/6/2025 and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days. ***Please submit the playground safety results for the drop test on the fall zones of 3 outdoor activity areas. ***The Director was reminded of the license expiration date and to log on to the portal to pay the fees prior to the license expiring. 3 of 3 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was emailed at the time of the inspection. The Emergency Disaster Plan update form was emailed at the time of the inspection. The DES Group Size requirements were observed to be compliant at the time of the inspection. The following was discussed but not limited to: 1. The playground must have a fall zone test to ensure compliance with the resilient surfacing requirements. 2. Staff files must be complete with all of the required documentation, including documentation of 2 good faith effort references. 3. The menu must reflect the specific items served to children. 4. Staff must record times of departure and arrival from their lunch breaks. 5. Cleanliness of the playgrounds. 6. Highchair trays must have smooth surfaces to ensure they are sanitizable. 7. Caulking around the sinks must be clean and free from cracks. 8. Diaper areas must contain soap, and water maintained between 86 - 110 degrees. 9. Exposed drywall must be repaired or covered so it is not accessible to enrolled children.

Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 5/10/2024, and are subject to changes pending programmatic review. Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed, but is not limited to: *The staff's starting date versus date of hire/employment date, as it relates to fingerprint clearance cards and annual training cycles, and *The Empower program is no longer subsidizing the fee. There were 4 staff files reviewed. 4 of the 4 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Assessment was completed at the time of the inspection.
