Noncompliance Area: While reviewing the children's files I observed that child 3 did not have who they could be released to on their agreement form.

Correction Required: An agreement shall specify the persons designated by a parent to whom the child may be released.

Noncompliance Area: While reviewing the children's files I observed that child 1, 2, did not have their parent's work information on the emergency contact sheet.

Correction Required: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent.

Noncompliance Area: While reviewing the children's files I observed that child 2 did not have the address for the person they could be released to on the emergency contact sheet.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: While reviewing the children's files I observed that child 1 emergency contact sheet was not updated.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information

Noncompliance Area: While reviewing the children's files I observed that child 3 did not have the special diet/meds field completed on their emergency contact sheet.

Correction Required: A child's record must contain signed parental consent for administration of medications or special dietary needs.

Noncompliance Area: While reviewing the children's files I observed that child 1 did not give consent on his emergency contact sheet for walking excursions, and so on.

Correction Required: A child's record must contain signed parental consent for transportation, walking excursions, swimming and wading.

Noncompliance Area: While reviewing the staff's files I observed that staff 1, and all members of the house hold did not have the NSOR certification.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 1 may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1 may not work in a child care position at the facility.

Noncompliance Area: During renewal inspection on Wednesday, 10/03/19, cert rep observed facility failed to have a written plan of daily activities and plan failed to be posted.

Correction Required: A written plan of daily activities and routines, including a time for free play shall be established for each group. The plan shall be flexible to accommodate the needs of individual children and the dynamics of the group. The written plan of daily activities and routines shall be posted in a traffic area used by parents.

Noncompliance Area: During renewal inspection on Wednesday, 10/03/19, cert rep observed facility failed to have a written emergency transportation plan identifies the means of transporting a child to emergency care and the facility staffing provisions. Plan also failed to be posted in child care space.

Correction Required: A written plan shall be conspicuously posted which identifies the means of transporting a child to emergency care and the facility staffing provisions in the event of an emergency. The plan must accompany a staff person who leaves the facility on an excursion with children.

Noncompliance Area: During renewal inspection on Wednesday, 10/02/19, cert rep observed facility failed to have copy of applicable regulation and regional day care office contact information posted.

Correction Required: A copy of the applicable regulations under which the facility is certified shall be posted in a conspicuous location used by parents, with instructions for contacting the appropriate regional day care office posted at the same location

Noncompliance Area: During renewal inspection on Wednesday, 10/02/19, cert rep observed facility's first aid kit failed to include soap.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: The operator has a handbook on location with all of the policies required. On 7/26/2018 the operator stated she ran out of copies of her handbook and did not provide all parents at time of enrollment with a copy of her policies as required.

Correction Required: At the time of enrollment, a parent shall receive in writing the facility's general daily schedule, hours for which care is provided, fees, responsibilities for meals, clothing, health policies, supervision policies, night care policies, dismissal policies, transportation and pick-up arrangements.

Noncompliance Area: The operator provided written incident reports in Spanish to a parent who only speaks English. The parent was unable to understand to incident reports and no person was available to translate for the provider who was unable to communicate the incident that occurred.

Correction Required: The operator shall establish oral or written communication in the language or mode of communication which is understandable to the parent.

Noncompliance Area: An unlabeled bottle was observed in the refrigerator.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Noncompliance Area: The emergency contact form and financial agreement form for child #1 and #2 have not been update for over a six month period.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.
