The provider had an infant in a swing and she had fallen asleep. The staff did not take the child out of the swing and place her in a pack n play for over 5 minutes. There was not a waiver for the child to be in the swing for sleeping. It is noted that the swing does lay down, it is not completly flat and there is break in the surface where the swing is able to be adjusted to a sitting position.
